Important information about

... signatures

Some forms require a certified signature. If your form says you need that, wait to sign it until you are with the person who will certify your signature. Follow the instructions on the form about having your signature certified. For more about who can certify, see Signature certification.

…death certificates or other legal evidence

If a form asks for legal evidence, such as a death certificate, be sure to include it. However, we won't return it. A photocopy is fine if the stamp or seal is legible.

…sending savings bonds to us

Some forms tell you to send the bonds with the form. Others tell you not to send in the bonds. Follow the instructions on the form.

If you are asking us to pay you for paper savings bonds, send the physical bonds (signed or unsigned) along with the form and mail them to us at the address shown on FS Form 1522.

FS Form 1014	Certificate of Incumbency of Officers (Corporation or Unincorporated Association)
FS Form 1048	Claim for Lost, Stolen, or Destroyed United States Savings Bonds Request payment or replacement of lost, stolen, or destroyed bonds. Sign this form in the presence of a notary or a certifying individual. HELP VIDEO: Locating your lost Savings Bonds To get the proper version of FS Form 1048, please answer the following question. Do you know your bonds’ serial numbers? Yes Use this FS Form 1048. No If your bond was issued before 1974, use this FS Form 1048. Use this version even though you don’t know the serial numbers. If your bond was issued in 1974 or after, go to Treasury Hunt. Enter the information Treasury Hunt requests. If the system finds your bonds, it will give you a special version of FS Form 1048 that enables us to process your claim without serial numbers. Make sure the form shows a reference number at the top, then complete the form. Sign it in the presence of a notary or a certifying official. Mail the form to the address on the form.
